General information

Autologous adipocyte grafting, more simply known as fat injection or lipofilling, is a major advancement in reconstructive surgery, especially in breast reconstruction. It is most often used alongside implant reconstruction or latissimus dorsi flap reconstruction, but it can occasionally be sufficient for a full reconstruction. In such cases, it allows the breast to be shaped in a harmonious way while avoiding the use of breast implants.

Technique

Fat cell harvesting is performed using a modified liposuction technique adapted to preserve the cells. The technique may seem simple, but it involves many subtleties, as damaged cells will not survive and the result will be disappointing. 

The precious harvested cells are purified and then grafted according to precise criteria into the recipient tissues of the new breast. The graft is performed through micro-incisions that allow the placement of living cells throughout all tissue layers. This means there is no residual scarring. 

The injection is performed in both superficial and deep layers, achieving a more natural result while respecting tissue integrity.

Procedure

Breast reconstruction by autologous adipocyte grafting, or breast reconstruction by fat injection, is carried out in several stages depending on the breast volume to be restored. The average number of procedures is 3. The number of sessions may vary according to each patient’s aesthetic goals.

This procedure is performed under general anaesthesia. The patient is admitted to the clinic on the morning of the operation and discharged the following day. The procedure lasts between 1 and 3 hours, depending on the complexity of fat harvesting and the extent of the donor areas. One of the strengths of this method is the short clinic stay, generally limited to 24 hours.

Postoperative period

One of the main advantages of this procedure is its minimal impact on daily life. Wearing a compression garment on the donor sites is recommended for 15 days, and a suitable bra for 3 weeks. Pain is minimal, and the required sick leave varies from 1 to 7 days depending on the case. Recovery is generally quick, with little discomfort and a fast return to normal activities. Postoperative follow-up is organised to monitor the progression of the results.

Results

The results of breast reconstruction by fat injection are immediate. The obtained volume is visible and very satisfying. However, swelling gradually decreases and about 30 percent of the grafted cells will disappear. The result stabilises after 2 months, once the swelling has resolved. At this point, a second complementary stage may be performed, as several sessions are usually necessary to achieve the desired result. This method offers long-lasting results and a harmonious effect thanks to the possibility of adjusting the breast volume and shape according to each patient’s expectations.

The main advantages of this method are : short clinic stay (24 hours), minimal pain, absence of scarring, preservation of breast sensation and softness.
