General information
Adipose tissue is composed of fat-filled adipocytes and a collagen structure with small blood vessels and nerve endings. Stem cells (undifferentiated cells that, under the influence of certain factors, can differentiate into muscle cells) have been discovered within these adipocytes. Since the 2000s, we have been able to harvest adipose tissue without damaging the cells and we can transfer living cells from one part of the body to another.
Indications
Age related loss of facial fat volume is the main indication for lipofilling. The injection of autologous fat cells not only restores volume to a hollowed face but also improves skin quality making it better vascularized smoother and less wrinkled.

Technique
After one or more consultations and photographic analysis with the patient the indication for lipofilling is confirmed. The area from which fat tissue is harvested must be determined; most often it is the abdomen or the inner surface of the knees and thighs. Fat is harvested through a very small incision allowing the introduction of a fine cannula. The fat tissue is gently aspirated into a sterile container and prepared for transfer to the face.
The cells are implanted in the face using a small cannula. The technique may seem simple, but many details must be followed, and the surgeon’s extensive experience is required for the graft to take successfully.
Each step is carried out with extreme rigor and care to guarantee graft take and a perfectly natural result.
Procedure
The ideal approach is to perform the procedure under general anesthesia. Local anesthesia is possible but less comfortable.
After marking the donor and recipient areas I gently harvest fat from the inner knees. The micro incisions are closed with surgical glue and a dressing is applied to the donor site. I then proceed to the face placing the fat cells precisely where needed using a very fine cannula inserted through needle sized entry points.
A small amount of surgical glue is applied to the micro openings and an anti bruising cream along with a cooling mask is placed on the face.
Postoperative care
The more areas treated the greater the swelling and bruising during recovery. At least one week is required before returning to work with light makeup. Sun exposure must be avoided for three weeks.
There is no pain but a sensation of swelling and concern that it may remain. The result stabilizes at around two months.
Results
Lipofilling provides natural and long lasting filling of facial hollows along with a noticeable improvement in skin quality making it fresher brighter and less creased.
When the treatment is performed to correct asymmetry or an abnormal hollow in a young patient the result can be considered permanent.
When treating aging the result remains durable however the natural signs of aging will gradually reappear over time.
