Autologous fat transfer to the buttocks, also known as buttock lipofilling, aims to rebalance the middle third of the body (pelvis and waist) by harvesting adipose tissue from one area and transferring it, after purification, to another. For example, fat may be taken from the abdomen and reinjected into the buttocks. This is a delicate procedure that requires impeccable technique and can deliver very attractive results.

This technique offers a dual benefit : it reshapes certain areas of the body through gentle liposuction while increasing buttock volume in a natural way. Unlike buttock implants, it uses your own fat, ensuring better integration and reducing the risk of rejection.

Indications

Buttock lipofilling is ideal for patients seeking a harmonious and progressive augmentation without resorting to implants. It is particularly suited to those who want a natural appearance and prefer to avoid overly invasive procedures.

The person wishing to undergo this procedure must have sufficient fat reserves, particularly around the waist, thighs, or knees. However, this technique is not suitable for very thin people.

Technique

The harvesting technique must meet the same requirements as any graft procedure : living cells must be preserved, which is why only gentle liposuction is used. The placement of purified fat strands into the muscle and beneath the skin must strictly avoid vascular zones, as the risk of fat embolism is significant if the technique is not perfectly controlled.

Depending on the patient’s needs, the amount of fat injected is adjusted to achieve a result that is both volumizing and natural. Buttock lipostructure makes it possible to obtain a harmonious contour while respecting each individual’s morphology.

Procedure

The operation is performed under general anesthesia in the operating room, with one night of postoperative monitoring and thrombosis prevention. While you are asleep, I harvest fat from the areas we previously marked together in your room before surgery.

These donor areas then evolve as they would after gentle liposuction. Adipocytes are extracted from the harvested fat and grafted in strands, using small rounded cannulas, into the superficial fibers of the gluteus maximus muscle and the hypodermis. Once the desired contour is achieved, an additional 30 % of volume is added to compensate for the predictable resorption of grafts, and the procedure is completed.

The entry points are closed with a drop of dermal glue. A specific compression garment is then applied as you begin to wake up.

Please note, I do not perform deep fat grafting into the gluteus maximus muscle, but rather superficial grafting. This avoids the risk of complications such as fat embolism, which can sometimes be fatal. Ultrasound-guided gluteal lipomodelling allows for the placement of fat grafts deep within the muscle for a more voluminous result. While this technique is very interesting from a purely surgical standpoint, I do not perform it because a small risk of fat embolism always remains.

Ultrasound-guided buttock augmentation with lipofilling theoretically allows for more controlled injection of fat thanks to real-time imaging, but this technique, although promising, does not completely eliminate risks and must be considered with the utmost caution.

Postoperative course

No pressure should be applied to the grafted areas, meaning the upper buttocks, for at least one week. The compression garment must be worn day and night for one week, then during the day only for an additional two weeks. Sleeping on the stomach is required for one week.

Pain is generally very manageable, but a week of rest is recommended, as the overall procedure can be tiring. Bruising and swelling gradually subside, revealing a more contoured and better-defined buttock shape.

Results

It is difficult to assess the results during the first two months, as bruising and swelling affect all treated areas. After two months, not only are the buttocks more shapely, but the skin quality is also significantly improved.

Buttock lipofilling provides a natural and harmonious result that integrates perfectly with your silhouette while preserving overall body balance. Even after this procedure, maintaining gluteal muscle tone remains important, as muscle mass naturally tends to decrease over time.
