Approach to dermatologic surgery

The practice of dermatologic surgery requires solid knowledge in dermatology, which makes it possible to avoid operating on harmless or spontaneously resolving lesions, as well as expertise in delicate surgical techniques such as those used in cosmetic surgery. It is therefore quite natural that, having worked in the dermatology department of Cochin Hospital under Professor Escande from 1998 to 2004, I developed this specialty.

Indications for dermatologic surgery

Skin surgery includes, on the one hand, the treatment of skin tumors and, on the other hand, the treatment of scars.

Skin tumors

Noticing a lesion on the skin can be a source of concern. Some lesions are completely benign, such as sebaceous cysts, lipomas or seborrheic keratoses, and only require treatment for aesthetic reasons or for comfort. Others, however, must be removed to prevent complications or malignant transformation. This is particularly the case for atypical moles, which require careful monitoring.

When removal is necessary, I perform a surgical excision, a precise procedure that eliminates the lesion with an appropriate safety margin. The intervention is quick and is generally performed under local anesthesia. The resulting scar is always designed to be as discreet as possible.

In the case of malignant tumors, such as basal cell carcinoma, squamous cell carcinoma or melanoma, management is more rigorous. A histopathological analysis is systematically carried out to ensure that all tumor cells have been completely removed. Depending on the location and extent of the lesion, skin reconstruction may be necessary. I then use appropriate techniques such as local flap surgery, skin grafting or cutaneous flaps to restore harmony to the treated area.

I always recommend regular dermatologic follow up, especially for patients with fair skin, a family history of skin cancer or prolonged sun exposure. Early detection often allows for simpler treatment and better outcomes.

Scar treatment

A scar is much more than a simple mark on the skin. It can be the reminder of an injury, an operation or a burn. Sometimes discreet and fine, it can also become thick, red, painful or, on the contrary, sunken into the skin. When a scar is bothersome, solutions exist to improve its appearance and restore suppleness.

Depending on its appearance and location, several approaches can be considered. Surgical revision makes it possible to remove an unsightly scar and close the skin with ultra fine sutures, for a more harmonious result. In some cases, a local plastic procedure allows the scar to be repositioned so that it follows the natural lines of the skin and becomes less visible.

Certain treatments can also complement surgery to optimize the result, or even replace it entirely. Hyaluronic acid injections can soften a depressed scar and restore volume. Specific dressings promote better healing and reduce the risk of inflammation. For an even more uniform appearance, laser sessions or dermabrasion may be considered, particularly to improve the texture and color of the scar.

Each skin reacts differently, and each scar tells a story. My role is to find the most suitable solution for each situation, to help the skin regain its balance and make the mark as discreet as possible.
