Monsplasty : Pubic Liposuction and Lipofilling

The mons pubis refers to the pubic area in women. In men, this region is simply called the pubis. Both men and women may feel discomfort due to an accumulation of fatty tissue in this area, especially when it is associated with skin laxity. This situation is most commonly observed after significant weight loss, when this region may require reconstructive surgery. In such cases, the procedure plays an important role in helping patients reclaim their body after distressing physical changes.

General information

This area of the body remains taboo, and is rarely discussed openly. Yet it has been widely depicted throughout art and pictorial representation, giving rise to a fairly clear artistic standard, most often hairless. Indeed, pubic representation (from pubes, meaning hair in Latin), such as Gustave Courbet’s The Origin of the World, was once considered scandalous.

Monsplasty is an intimate surgical technique that helps restore a harmonious silhouette and daily comfort by addressing excess fat or skin laxity in the pubic area.

Indications

The procedure is most often recommended in cases of discomfort, particularly when sitting for long periods or during sports activities. In some cases, the concern is primarily aesthetic. This surgical intervention is also well suited for patients who have experienced major weight loss and wish to regain a more balanced appearance. It may be combined with other intimate procedures, such as nymphoplasty, to achieve a global and harmonious result.

Technique

There are three main techniques: liposuction, skin tightening, and autologous fat grafting. Each method is selected according to the patient’s specific needs and morphology to ensure a natural and long-lasting result.

Pubic liposuction removes excess fat, while pubic lifting restores firmness to lax skin. Pubic lipofilling is ideal for restoring volume in cases of deficiency and reshaping the area in a more harmonious way. These techniques can be adapted and combined depending on the patient’s anatomy and expectations.

Procedure

The procedure is performed under local or general anesthesia, on an outpatient basis, with one to three hours of postoperative monitoring.

Pubic liposuction reduces excessive bulging by removing excess fat. It is most often performed in a medico-surgical office under local anesthesia and lasts approximately 30 minutes.

Skin tightening or pubic lifting may sometimes require general anesthesia and is then performed in a clinic, still on an outpatient basis. The scar is discreetly hidden within the underwear line and may be combined with liposuction.

Autologous fat grafting is indicated in patients with insufficient volume, particularly in slender individuals. In this case, the volume of the mons pubis is increased, often at the same time as the labia majora. A small amount of fat is harvested, for example from the inner knee, and reinjected using fine cannulas that leave no visible scars. In addition to restoring volume, this technique also rejuvenates the overlying skin.

This level of precision is only possible through careful listening, a clear aesthetic vision, and meticulous execution.

Postoperative care

Postoperative recovery is generally simple and painless. Bruising may occur after liposuction or fat grafting and usually resolves within 15 to 21 days. Sexual intercourse should be avoided, and prolonged sitting may be uncomfortable during the first 15 days following surgery.

In cases of skin excision, resorbable sutures are used and do not require removal. Showering is allowed from the day after surgery.

Results

It takes approximately six weeks to fully appreciate the results, once postoperative swelling has subsided.

A pubic area that was previously too visible under certain clothing no longer causes discomfort, the fold above the genital area disappears, and the overall appearance becomes smoother and more aesthetically pleasing. Results are long-lasting and contribute to renewed self-confidence and an improved quality of life.
