The first calf implants were developed to treat patients with post-polio sequelae, who most often presented with atrophy or hypotrophy of one calf. The goal was to restore symmetry by using the healthy leg as a reference. Given the quality of the results obtained, the procedure was later extended to less severe cases with the same aesthetic objective : achieving a symmetrical and harmonious calf contour.
This procedure can also be combined with other techniques to enhance the overall silhouette. A liposuction of the ankles or knees can refine the leg contours and accentuate the calf shape. Likewise, buttock lipofilling may be considered to rebalance the entire lower body and further enhance the silhouette.
Indications
The first indication remains post-polio sequelae, which have become very rare thanks to vaccination.
The second and most common indication for calf augmentation is non-pathological underdevelopment of the calf muscle, specifically the gastrocnemius muscle. The inner portion of the gastrocnemius is what gives the calf its shape, especially when viewed from the front. A lack of inner contour can even create the impression of bowed legs.
In some cases, the goal is to correct an asymmetry, when one calf is less developed than the other. Other patients seek a purely aesthetic improvement, wishing to achieve more sculpted, well-defined, and better-proportioned legs. Thanks to advances in aesthetic leg surgery, it is now possible to obtain a natural and harmonious result, tailored to each patient’s needs and expectations.

Technique
The choice of technique depends mainly on the volume to be restored. When the correction required is moderate, lipofilling is preferred. This technique involves harvesting fat from another area of the body, usually the abdomen, thighs, or knees, using gentle liposuction. After purification, this fat is precisely reinjected into the calf, layer by layer, in order to create a natural and harmonious shape.
If the volume deficit is more significant, silicone calf implants are necessary. These are inserted against the muscle through a small, discreet incision in the natural crease of the popliteal fossa behind the knee. This approach allows for greater volume, with a long-lasting and stable result. During the consultation, volume trials are performed to better visualize the final result and choose the solution best suited to the patient’s expectations.
Calf lipofilling requires great precision in injection to achieve aesthetic, natural results that blend seamlessly with the silhouette.
Procedure
Under general anesthesia in the operating room, the procedure lasts approximately 1 hour and 30 minutes. For lipofilling, the first step involves harvesting fat via gentle liposuction, before gradually reinjecting it to sculpt a natural and symmetrical shape. For implants, an incision is made behind the knee to place the prosthesis against the muscle, respecting the natural proportions and lines of the leg.
At the end of the procedure, compression stockings are applied to optimize recovery and limit swelling, as well as a light compression garment if fat has been harvested from the abdomen.
Postoperative course
The first night after surgery is spent in the clinic to ensure optimal monitoring and effective pain management. Walking is resumed the same day. You should allow between 5 and 15 days of rest, one month before resuming sports.
Results
The result is visible immediately, with progressive improvement over the following weeks. The calf regains a more defined and harmonious contour, better integrated with the overall shape of the leg. After two to three months, swelling has completely resolved and the final volume stabilizes.
The leg appears more balanced, with better ankle definition. The entire lower limb appears straighter.
