The idea of harvesting fat from one area of the body to transfer it to another is not new. Numerous attempts have been described since the end of the 19th century. However, the technique was not reliable until the technical advances developed, notably by Sidney Coleman in 1995 (see technical sheet lipostructure SSCPRE and SOFCPRE).

The fat transfer technique must be perfectly mastered for the procedure to be successful. Having completed specialized training in the early 2000s, I have personally performed this procedure for over 15 years with excellent results. Recent technological advancements are integrated into my practice to guarantee the best possible outcomes.

Before / after results of breast lipofilling

Results are stabilized after 4 months. It is important to understand that the fat cells injected into the breasts will evolve like the other fat cells in your body : if you gain weight, they will increase in volume. Conversely, if you lose fat, for example through intense physical activity, breast volume may decrease.

For a stable result, your weight should remain stable.

Ultimately, your breasts will evolve just like natural breasts.

Indications for breast augmentation by lipofilling

The best indication for breast augmentation by lipofilling is a woman with small breasts and localized fat deposits on the hips or thighs. In this case, both concerns are treated at the same time.

Other excellent indications include :

  • Breast asymmetry
  • Tubular breasts
  • Discreet loss of volume associated with an increase in waist circumference in post menopausal women

It’s also a very good procedure for women who want to have their implants removed while minimizing volume loss. The procedure is more delicate, but it’s possible to replace the implants with fat tissue.

Technique of breast augmentation by lipofilling

The technique must absolutely be adapted to each patient’s morphology and lifestyle habits. This is why there will always be several techniques, each with its own advantages and disadvantages. My technique is derived from Sidney Coleman’s description and adapted for breast surgery, as taught to me by Dr. Emmanuel Delay.

It consists of three steps :

  • Harvesting fatty tissue while preserving the survival of adipocytes and stem cells through gentle aspiration.
  • Purifying the harvested tissue to retain only the implantable cells.
  • Implanting these cells using thin ribbons placed in the target area so that the cells can find a new anchor and the graft takes at over 70%.

Procedure of breast lipofilling surgery

A light general anesthesia is used, and the procedure lasts between one and two hours, on an outpatient basis. To increase breast size by the equivalent of one cup size (approximately 150 ml), it is necessary to harvest around 1,500 ml of fat from the body, which extends the operating time in patients who do not have a localized fat reserve.

In cases where you do not have localized excess fat (no saddlebags or excess belly fat), liposuction should be performed in small quantities, injected in droplets forming thin ribbons in the mammary gland, around the breasts and in the décolletage.

Post operative care after breast lipofilling

Bruising may occur in all treated areas, but pain is usually minimal. It is recommended to plan one week off work.

Follow up consultations are scheduled twice during the first month, then again after 4 months. At that time, a second session may be considered, sometimes planned from the outset, in order to achieve greater volume.

Breast lipofilling is less invasive than implant based breast augmentation. Unlike augmentation with implants, no foreign bodies are involved, which reduces the risk of complications.

In addition, the benefits are double : while increasing breast volume, fat is removed from another area of the body, such as the abdomen, hips, or thighs, refining the silhouette.

FAQ

To undergo breast augmentation by lipofilling, breast growth must be complete. The procedure is therefore not recommended before the age of 18.

Breasts augmented by lipofilling age very naturally since the fat comes from the patient. This fat may therefore be absorbed or increase over time, potentially leading to a change in volume. A touch up can be considered if necessary.

Rare but possible risks include :

  • Fat calcifications or micro calcifications
  • Volume or shape asymmetry between the breasts
  • Infections
  • Development of fat cysts

Choosing an experienced surgeon is essential to minimize these risks.

In the months following surgery, part of the injected fat may be reabsorbed. It is therefore necessary to wait at least 6 months to assess the final results. Once stabilized, the results are irreversible.

If the operation is not due to trauma, malformation, or cancer, the “illness” value must be proven to the insurance company in order to obtain coverage.
